CSA Crime & Incident Reporting
Do not use this form if a crime is still in progress or there is an immediate threat to the community. Instead, call Public Safety at 781-239-5555 or x5555.
This form is to be submitted ONLY by a designated Campus Security Authority (CSA), to report to the Babson College Public Safety Department allegations of crimes that were reported to the CSA. CSAs should complete this form to the best of their ability as soon as possible after the incident was reported to them.

According to the Higher Education Act, or 20 U.S.C. 1092, now known as the Jeanne Clery Disclosure of Campus Security Policy and Campus Crime Statistics Act, Babson College is required to collect and annually publish statistics concerning the occurrence of specific crimes on campus property (including residential facilities), noncampus property, and public property, reported or known to the Public Safety Department, local law enforcement or any individuals identified by the College as “Campus Security Authorities” (CSAs). Campus Security Authorities are defined as, “An official of an institution who has significant responsibility for student and campus activities, including but not limited to, student housing, student discipline, and campus judicial procedures.”
Campus Security Authorities (CSAs) are federally mandated to report to the Babson College Public Safety Department if they learn about potential Clery Act crimes/incidents. CSAs should not seek to investigate or determine if a crime is Clery reportable. For Babson College to satisfy the statistical reporting requirements of the Clery Act, all CSAs are required to complete this form when allegations of specific crimes/incidents are reported to them. If there is no immediate threat and person(s) who reportedly experienced gender-based misconduct (Sexual Assault, Rape, Fondling, Domestic Violence, Dating Violence, Stalking) do not wish to report an incident to Public Safety/law enforcement, the CSA should not submit this form or otherwise report the incident to Public Safety, but should as required by College policy promptly report information about this matter to Babson Title IX Office, via 781-239-5501 or brauch1@babson.edu.
Babson uses the information collected from these forms to prepare a compilation of statistical crime information for inclusion in the College’s Annual Security and Fire Safety Report. Babson College Public Safety will use information reported through this form to determine the category of the crime/incident and the location under which the crime/incident should be reported according to the requirements of the Clery Act. Data collected on this form is to be used to promote crime awareness and enhance campus safety.
It is the policy of Babson College to ensure that victims and witnesses to crime are aware of their right to report criminal acts to law enforcement, and to report College policy violations to the appropriate office (e.g., student conduct violations to the Office of Community Standards). However, if a reporting person requests anonymity, this request must be honored by omitting from this form any identifying information about the reporting party and alleged involved parties. No information should be included on this form that would personally identify a victim without their consent.
